Informed Consent, Liability Waiver, Media Release for Prana Life Works

By participating in the classes/practices accessed through or provided on this website, I acknowledge that I am voluntarily engaging in the activity and accept any and all risks associated with my decision to participate. I represent and warrant that I am physically fit and have no medical condition that would prevent my participation.

I understand I may be provided information and guidance about yoga, related mindfulness practices, health, and lifestyle wellness. I understand that I may be increasing my physical activity levels and that there are inherent risks involved with any and all movement and exercise programs, including the possibility of muscle soreness or strains, physical injury, and a physiological reaction or event. I am fully aware of the risks and hazards involved and agree to take responsibility for my unique needs by engaging a pace that feels comfortable to me, stopping immediately if needed, and seeking professional medical advice and support as needed.

I understand that Prana Life Works and its affiliated yoga professionals are not medical professionals, nor are the classes, practices and programs a substitute for medical or mental health care. As with all forms of physical exercise, it is recommended that you consult with a physician before starting a new exercise program and follow their safety recommendations.

I understand that classes may be recorded and that I can decide to show my image on video or not. If I choose to show my image on video, I grant permission to use my image in media including, but not limited to online marketing, social media, emails, video recordings.

I hereby waive any right to inspect or approve the electronic matter that may be used in these publications now or in the future, whether that use is known to me or unknown, and I waive any right to royalties or other compensation arising from or related to the use of the image.

I hereby forever waive, release and discharge Prana Life Works and its affiliates and representatives from any and all claims and liabilities for injuries or damages to my person and/or property, including those caused by negligent act or omission, arising out of or connected with my participation in any activities or services.

I have read and fully understand the above advice and information. I voluntarily agree to the above Informed Consent and Liability Waiver and Media Release under my own free will. I understand the above Informed Consent Liability Waiver and Media Release also apply to any individuals that I invite to participate with me on my account. Checking the box below serves as complete and unconditional release of all liability to the greatest extent allowed by law.
